To give some context of what is happening, criticalcon wrote an entire backstory for this picture. Be warned, it's long.

Amy and Sally are already married and living together in a luxurious cabin/cottage near Sally’s main palace in Knothole (she IS an on-call princess, after all). One day, Amy is standing near the fireplace in the living room, looking over different pictures of the two of them with all their friends and reminiscing about all the fun and zany adventures they’ve had together over the years, as well as how much they’ve all changed and matured over time. (Sally, meanwhile, is sitting in a nearby chair, trying not to get a headache over the latest batch of citizenry requests and monthly royal-budget paperwork strewn across her lap.)
While on this specific train of thought, Amy comes across one of her old pictures of her and Sonic, having a picnic near Green Hill and smiling for the selfie. At the present, Amy smiles at this picture at first of the two of them in younger days—until she takes a closer look at it and realizes just how forced Sonic’s smile really is, being that he hadn’t had a choice in the matter on this particular occasion and this was still the time of her life when Amy was ultra-obsessive about trying to date Sonic and get him to hang out with her in any way she could, his input or feelings be damned. Kicking herself in the present for only just now noticing as much, Amy starts to have other memories flood back: Sonic reaching his breaking point in a Station Square bistro and publicly embarrassing both of them by letting his true feelings out for her for all to hear (i.e., you’ve always been a friend to me, but JUST a friend; any romantic attraction you have for me is strictly a one-way street; I’m sick and tired of you trying to force yourself on me like this—grow up already and find someone else who’ll give what you’re looking for); the cold rejections she’d made to those of Sonic’s friends who WERE interested in her in the years leading up to that point (Blaze, Silver, and—of all people—Shadow among them); some of the nasty and petty things she’d said to Sally when she and Sonic has dated for a while, in part out of bitterness and in part to keep running as far as she could from the truth of her own sexuality (in this timeline, she’s pansexual) and her own attraction to Sally.
This last thing leads her to put the picture back down on the mantelpiece, tears now streaming down her face as she begins to sob—over losing Sonic, over losing any number of other wonderful husbands or wives she could have had, over the cowardice she’d displayed for too long both in running from who she really was and putting her own interests and feelings ahead of those of others, & over the number of friendships she’d damaged over the years that took years for her to untangle and undo.
Seeing her wife sobbing and into her own hands, Sally gets up and moves to Amy’s side, gently lifting her head up and asking her what’s wrong. After spilling all of the above information in a torrent of words and apologies (especially to Sally for all the awful things she ever said or did to her), Sally (and this is what I’d want the picture itself to actually be) locks her in a tight embrace, fighting back some tears of her own, and eases Amy’s tear-soaked face against her chest, stroking her quills with one hand and her back with the other. All the while, she whispers many reassurances into Amy’s ear to calm her down: she’d already forgiven her a long time ago for those words and actions; the Amy of those younger days is not the same Amy she is now, and even then, that younger Amy was merely young, love struck, stupid, and afraid more than outright malicious, so there’s no need to hate herself for who she used to be; she’s grown and matured so much since those days and she’s REALLY proud of her for that, regardless of what anyone else says or thinks; she’s ALWAYS thought of Amy as a beautiful soul even during those times when they were strained from each other, and nothing Amy ever said/says or does/did will change that or the love she has for her.
Now reassured and with her tears trailing off into congested sniffles, Amy smiles a little as she continues to lean her head into Sally’s chest for a while before lifting it up to look at Sal’s soft smile, to which Sally leans down and exchanges a long, sweet kiss with her love, after which Amy thanks her and tells her she loves her, with Sally responding in kind. Smiling, Amy puts her head back into Sally’s chest and Sally gently rocks the both of them back and forth for a while longer. At length, Sally asks if Amy would like to get rid of that particular picture. Surprised, Amy wonders aloud why Sally would want to, aside from how sad it had just made Ames. Sally, with a wry smile, replies that she has her own memories of failure she’d like to clean out, musing on how her own relationship with Sonic fell apart because they were too similar to each other—exacerbating each others’ weaknesses as well as strengths tenfold, thus holding up a virtual mirror of each other neither had been quite prepared to view—and not, she’s quick to assure Amy, because of anything she said or did. Now smiling as well, Amy asks if perhaps they should ask Sonic and his husband (in this timeline) Silver out for a double-date picnic, in part to hang out & catch up with them and in part to make suitable replacement for the picture, to which Sally replies that she thinks that would be a wonderful idea.
